I am doing a limited number of medical massages and I do prefer a DOCTOR'S NOTE, PRESCRIPTION, or a LETTER OF REFERRAL. The note should state your doctor's name and the referral date. ***If you don't feel well, or have a cough, breathing issues, common cold/flu-like symptoms, runny nose, or not quite 100%, please stay home and don't travel--stay safe I don't have a debit/credit machine, but you can do cash/etransfer, and get a receipt for HEALTH INSURANCE SUBMISSION or I direct bill them. I am registered with NHPC and I have been practicing for 8 years. ****heated table is optional **** Sometimes I do take days off. If anyone wants earlier than this, I need to know the day before. *I can direct bill MOST health insurance companies. Approximately, 30 or more! I give receipts for HEALTH INSURANCE submission only!
