Just wondering... If I want a topless massage with a release, I'd normally pay an extra $20 to the MPA. Should I tip in addition to that, or is that considered the tip? If I should tip in addition to the extra, how much?

Do these exist at all? I have a health plan that covers massage, and would need a receipt for that part...but would like to pay cash on top for some extras.
